• Death certificate of the holder with a readable QR code (If we do not have it, we will guide you on the call)
• Official legible identification of the holder
• Current and legible official identification of the beneficiary
• Proof of address of the holder
• Proof of current address of the beneficiary (not older than 3 months)

Important

• In the event of accidental death, Record of the Public Prosecutor's Office with a description of the facts in the event that death is derived from an accident or violent event

Specifications:

• The Public Ministry certificate will only be requested when the death certificate mentions that it is homicide or suicide and the client has been a client for less than 1 year TeChreo
• For the loss claim, all documents will be submitted 100% digital. No photographs accepted.

For Accidental Death:

• Declaration of the Event to the Company (free format)
• Official identification of the Insured and in the case of being a foreigner, a document proving his legal stay in the country
• Death certificate of the Holder
• Official identification of the Beneficiary and, in the case of being a foreigner, a document proving his legal stay in the country
• “Proof of address of the Beneficiary not older than 3 (three) months;”
• "In case of being the Insured's spouse, a copy of his or her marriage certificate."
• "In case of being the son of the Insured, a copy of his birth certificate"
• If the beneficiary is a parent, the owner's birth certificate is requested
• Proceedings before the Public Ministry, in the event that the death of the Insured has occurred violently

For Daily Rent for Hospitalization due to Accident or Illness

• Declaration of the Event to the Company (free format)
• Official identification of the Insured and in the case of being a foreigner, a document proving his legal stay in the country
• Hospital receipt (invoice or hospital statement specifying the days of hospitalization)

For General Surgical Intervention

• Declaration of the Event to the Company (free format)
• Official identification of the Insured and in the case of being a foreigner, a document proving his legal stay in the country
• Hospital receipt (invoice or hospital statement specifying the days of hospitalization)
• Diagnosis and medical history
